Description:  Responsible for the development, implementation, coordination and supervision of the Foster Grandparent Program.

Multidimensional Family Therapy (MDFT) Therapist

Location: Racine Behavioral Health Services – 1212 Taylor Ave Racine, WI

Hours: M- F -Maintain flexible work schedule to adequately meet program needs, which may include working during the evening and on the weekend.

This position involves the care and treatment of adults and adolescents diagnosed with a mental illness or substance use disorder. This position is specific to the Multidimensional Family Therapy (MDFT) model. Duties include building foundation for change, facilitating change with individual and family, and solidifying the changes made within the MDFT model. Reports directly to the Mental Health Professional. Works with other case managers, consulting psychiatrist, front desk staff, director of quality assurance, consumers and managers. Has outside contact with community agencies, consumer family members and various hospital units.

  1. Participate in live supervision including video recording.
  2. Follow service plans for clients on his/her caseload, under the supervision of the Mental Health Professional.
  3. Participate with agency staff and community resources to support and implement service array.
  4. Maintain a treatment oriented environment and assure consistency in carrying out service plan objectives

REQUIREMENTS:

  • Master’s Degree in Social Services
  • 1 year of experience working with adults and adolescents who have a mental illness or substance use disorder.
  • Must become certified as an MDFT therapist and maintain certification yearly.
  • Must have Substance Abuse Counselor in Training (SAC-IT), Substance Abuse Counselor (SAC), or Clinical Substance Abuse Counselor (CSAC).
  • Must have ability to communicate effectively orally and in writing, and interact effectively with clients and other health care professionals.
  • Must have good communication and organizational skills.

Outpatient Mental Health Specialist 

Location: Racine Behavioral Health Services -Racine, WI
Hours:  M-F:  8AM to 4:30PM Variable Schedule to meet needs (40Hrs)

Description:  Provide counseling to individuals presenting with mental health concerns, often accompanied with substance abuse issues.  Provide clinical assessment and document bio-psycho-social intake after initial session.

Work Environment: This position offers excellent work/life balance, with a variable schedule to meet needs. Monday through Friday 8-4:30.

REQUIREMENTS:

  • LCSW
  • Master’s degree in Social Work

Information and Assistance Specialist 

Hours: M-F 8AM -4:30PM
Location: Racine ADRC- Sturtevant, WI

The Information & Assistance Specialist will provide a central source of reliable and objective information about a broad range of programs and services for older adults and people with physical or developmental/intellectual disabilities. Help people understand and evaluate the various options available to them.  Help people to find resources in the community and make informed decisions about long-term care, help people conserve their personal resources, maintain self-sufficiency and delay or prevent the need for potentially expensive long-term care.  Provide assessment and enrollment counseling for publicly funded long-term care, including Family Care and IRIS. 

REQUIREMENTS:

  • A Bachelor of Arts or Science degree (preferably in a health or human services related field) or a license to practice as a registered nurse in Wisconsin pursuant to s. 441.06 Stats.
  • Equivalent of at least one year of full-time experience in a health or human service field, working with one or more of the client populations served by the Aging and Disability Resource Center (elderly or adults with physical or developmental/intellectual disabilities).  Qualifying work experience may be paid or unpaid and may include internships, field placements and volunteer work.
  • Valid Wisconsin driver’s license.
  • Prompt and regular attendance.

RN Community Support Specialist

Location: Racine Behavioral Health Services – 1212 Taylor Ave Racine, WI
Hours: Mon-Friday- 8AM to 5PM

The RN - Community Support Program diagnoses and treats human responses to actual and potential health concerns.

Essential Job Functions: 

  1. Provide clients with an initial nursing assessment to identify health problems and to screen for communicable disease within 30 days of admission to CSP.
  2. Ensure adherence with CSP regulations that there are written orders on file for any prescription medications.  
  3. Ensure that all medication monitored by CSP has a label permanently attached to the outside of the container, which identifies its contents. Ensure all aspects of medication handling are in accordance with CSP requirements. Monitor quality assurance related to medication monitoring.
  4. Set-up medication trays and administer IM medication to clients.
  5. Educate staff on health requirements.  

REQUIREMENTS:

  • Bachelor’s Degree in Nursing (preferred) plus current RN Licensure.
  • One year of experience working with adults who have a chronic mental  illness.
  • Must demonstrate clinical competence in standards of nursing practice and ability to work independently in area of expertise.

License or Certificate:

  • RN Licensure
  • Certification as a generalist on psychiatric nursing is recommended but not required.
  • Must own and be able to legally operate a vehicle and have necessary insurance.

Clinical Supervisor-CSP

Location: Racine Behavioral Health Services – 1212 Taylor Ave Racine, WI
Hours: Mon-Friday- 8AM to 5PM

This is a management position encompassing overall clinical management of the Community Support Program (CSP) and Comprehensive Community Services (CCS). Duties include training, supervising and evaluating CSP / CCS staff and maintaining clinical responsibility of the program.

Reports directly to the CSP / CCS Director. Works with case managers, registered nurse, consulting psychiatrist, pharmacy, front desk staff, director of quality assurance, consumers and senior management team. Has outside contact with community agencies, consumer family members and various hospital units.

REQUIREMENTS:

Must have a Master’s Degree in Social Services plus 3000 hours of experience working with adults who have a chronic mental illness, or 1500 hours of experience in a CSP.

Must be a Licensed Clinical Social Worker (LCSW) in good standing with the state of Wisconsin.

Experience with treatment programs involving adults with chronic mental illness.

PHYSICAL/SENSORY DEMANDS:

Work is mainly in an office setting but will be also include work in the community (client’s homes, other community settings). Hours of work will vary (primarily regular business hours but will include some evening and weekend hours) and average at least 40 hours per week.

2. Prefer 2 years of related work experience in a recreational or human service field.

3. Obtain and maintain First Aid/ CPR Certification
